Our Goal

Our goal is to provide caring support and solutions to you and your loved one to reduce worry and stress when confronted with the challenging decisions of complex individual health needs and health systems.

Striving to improve the quality of life and dignity of seniors

  • We begin by performing a Detailed Needs Assessment to determine what is working and where there needs to be additional support or changes. A needs assessment consists of: 

    • A health history

    • Current medical issues

    • Cognitive screening

    • Medication review

    • A home safety assessment

  • Based on the outcome of the Detailed Needs Assessment, we will create a care plan that meets the medical and psychosocial needs of your loved one.  Explore the list of services we provide. If you don’t see what you think you need, please ask because your input matters!

SERVICES

We use a HIPAA compliant documentation software that keeps sensitive client information safe and secure. Families and loved ones can access this information through their own login for a nominal monthly fee.
