  • Also known as an Aging Life Care™ Geriatric RN Care Manager, is a holistic, client-centered approach to caring for older adults facing ongoing health challenges. Working with families, the expertise of an Ageless Transitional Professional provides the answers at a time of uncertainty. Their guidance leads families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress, money, and time off work for family caregiving.

  • Many Care Managers are social workers and don’t have the expertise of having a medical background. A licensed Nurse Care Manager can provide medication management and set up, management of chronic illnesses (cardiovascular, diabetes, etc), easily build a positive rapport with other healthcare providers.

  • If you are caring for or concerned about:

    -Concerns about the safety of the current living environment

    -Has multiple medical or psychological problems

    -Has declined in function, i.e recent falls, missed medical appointments, not taking medications, recent multiple ER visits

    -Is not pleased with the current care providers and requires advocacy

    -Has limited or no family support

    -Is being discharged from the hospital or rehabilitation center and needs a plan of care to return home

  • If the family or loved ones:

    -Needs education and guidance with assisting the individual and available resources in the community

    -Lives far away and has limited knowledge and time to help the individual

    -Is feeling overwhelmed with caregiving and confused about solutions and has limited knowledge about their loved ones chronic care needs

    -Requires directions and educational resources in dealing with behaviors related to dementia

  • Services are offered in a variety of settings. Professionals can serve the needs of their clients and their families by recieving:

    -Personalized and compassionate service - focusing on the individual's wants and needs.

    -Peace of mind that there is a professional RN care manager keeping watch over your loved one

    -Giving the family relief from constantly managing their loved one’s care, medical appointments, etc. You have more enjoyable time with your loved one.

    -Accessibility - care is typically available 24 hours a day, 7 days a week.

    -Continuity of care - communications are essential and coordinated between family members, doctors, allied health, and other professionals and service providers.

    -Cost containment - inappropriate placements , duplication of services, and unnecessary hospitalizations are avoided.

    -Quality control - aging life care professionals follow ALCA’s Standard of Practice and Code of Ethics.
