Comprehensive health care & home safety assessment

We will provide a detailed report with medical and non-medical recommendations reflecting appropriate level of care needs and solutions, working in partnership with the client and family.

Medication management

As a Registered Nurse, I will review medications and set up weekly medications (or as determined), coordinate refills and prescription renewals. I will act as a liaison between physicians, pharmacy, senior facility, insurance companies, and client/family.

Coordination of in-home services

We will assist with locating reliable and appropriate medical and non-medical services based on the level of care needed.

Medical & non-medical monitoring in lieu of family members 

Many aging family members live far away from helpful relatives.  Even if they live close, they will likely have kids, identifying them as the “Sandwich Generation”, therefore may not have the time or resources to dedicate to their loved ones needs. We provide in-person and limited remote monitoring for clients.  We also monitor adherence to medical orders, treatments, and overall safety at home.

Advocacy

As a Registered Nurse I will accompany clients to medical, legal, and other appointments when the client needs assistance with communication, physical support, etc, and clarify doctor's instructions. We can also attend facility care plan meetings representing family members and clients to provide support, advocacy, guidance, and education as needed to ensure excellent care and quality of life.

Assistance with coordinating transitions from different levels of care

We can help with the transition from home to a senior retirement ensuring that your loved one is getting the services and level of care that is needed and also meets their social needs. We can assist with the discharge planning from the hospital or rehabilitation center by making sure your loved one has all of the necessary information, services, and discharge instructions are followed such as follow up appointments.

Crisis prevention strategies 

Sometimes care needs can change quickly, having a plan in place can reduce stress and worry, allowing you to spend more enjoyable time with your loved one and less time dealing with a crisis.  

Remote consulting services for families or loved ones

If you or your loved one don’t know where to start and need guidance and don’t reside in the in-person treatment area of Ageless Transitions, we would love to offer guidance.  We can do the research and report back ideas and resources for your area. 

Assistance with senior housing

I have extensive past experience working in the senior housing industry both as an Executive Director and Healthcare Administrator.  We know how to navigate which senior housing place will meet your health and social needs. We can coordinate tours and accompany on visits of senior living communities, providing feedback and recommendations to help ensure the best possible choices are made.

Community resources

We have a strong working relationship with many professionals in the area, including non medical needs, such as trustees and elder law attorneys. 

Assistance with advance

directives & end of life care

When the time comes, we work closely with hospice to make sure your loved one is comfortable and receiving the best care in the right environment. End of life care can be difficult.  By having a liaison between you and hospice gives you the emotional space to be with your loved one and grieve.
